Nashville, Tenn. – The Tennessee Bureau of Investigation yesterday arrested a Springfield, Tenn. woman on multiple counts of obtaining prescription pills illegally after she was indicted by the Sumner County Grand Jury on January 6, 2012.
Ann Margaret Choate, 30, of 3833 Cross Plains Road, Springfield, Tenn., was indicted on 11 counts of obtaining drugs by fraud, nine counts of forgery and one count of identity theft. From December 2010 through May 2011, Choate was employed as an office staffer for a doctor’s office in Nashville, Tenn. While employed there, Choate stole prescription pads from the office and wrote herself multiple prescriptions for Lortab some with refills. Choate was having the prescriptions filled at a pharmacy in White House, Tenn. TennCare OIG also charged Choate three counts of TennCare fraud.
Choate was booked into the Sumner County jail and released on a $7,500 bond.
